Voluntary Centre Services is looking to recruit a specialist Mental Health focused Social Prescribing Link Worker to cover the Lincoln City area.
The Social Prescribing Service, developed alongside the Integrated Neighbourhood Working (INW) programme provides a means of enabling primary care services to refer patients with social, emotional or practical needs to a range of local, non-clinical services.
The new team member will provide support to patients alongside our existing Social Prescribing Teams in the Apex, Lincoln Healthcare Partnership and Lincoln South PCNs and Integrated Place Based Teams.
Social Prescribing Link Workers are responsible for delivering a person-centred service, supporting people to improve their health and wellbeing by connecting them with services in their community. Link Workers are a vital community resource, responsible for patient engagement and developing effective working relationships with health care professionals and voluntary/community sector providers.
This role involves regulated activity and requires an Enhanced level DBS check.
Salary £23,898 per annum
Flexible 28-35 hours per week
Annual Leave entitlement 29 days per year (pro rata)
